    Symbria Rx Services Great Plains is currently seeking a Staff/Consultant Pharmacist to add to our growing Pharmacy team.     Position Summary


    This is a blended role, including responsibilities as a Staff Pharmacist and a Pharmacist Consultant. This role will provide pharmacy services consistent with all applicable policies, procedures, legal requirements and safeguarding patient safety and effectiveness of therapy. This role will also provide pharmacist consulting services to long term care communities by providing education and audits to ensure the nursing facility is meeting regulatory requirements. (50% of the time consulting, 50% covering shifts in the pharmacy.  For the shifts they will vary between 8am and 10pm Mon-Fri with rotating every 3-4 weekends from 8am-6pm)


    Staff Pharmacist Functions and Responsibilities

    • Interpret and clarify new and refill order requests within accuracy guidelines to insure appropriateness and maintain patient safety.
    • Prepare complete medication orders within accuracy guidelines.
    • Responsible for final check on all prepared medication to insure completeness within accuracy guidelines; provide assistance as needed.
    • Prepare and complete all appropriate documentation including controlled drug sheets, stat reports, as necessary.
    • Monitor appropriateness of therapy based on patient status, lab work, culture results and clarify inappropriate physician orders when necessary.
    • Provide physicians, nursing staff, and public with drug information such as drug selection, dosage, proper administration, etc.
    • Provide “On Call” coverage as needed.

    Consultant Pharmacist Functions and Responsibilities

    • Drug Regimen Review (DRR).
    • Meet upon entry and exit (if possible) with Director of Nursing and/or Assistant Director of Nursing.
    • Meet upon entry (if possible) with Administrator and/or Assistant Administrator.
    • Make recommendations to the appropriate members of the Inter-Disciplinary Team to:
      • Ensure and enhance the quality of care for the resident in the consultant’s professional opinion.
      • Provide guidance toward adherence of laws, rules, guidelines, and policy and procedures from a pharmacy perspective.
    • After the DRR visit is complete, consultant recommendations (for physician and nursing review) and summary of recommendations should be left with the Director of Nursing or their designee at the facility.
    • Attend Quality Assurance (QA) meetings and provide QA reports:
      • QA report must minimally contain the following items:
        • Changes to emergency and/or contingency supply.
        • Antipsychotic chart and graph.
        • Antidepressant chart and graph.
        • Anxiolytic chart and graph.
        • Hypnotic chart and graph.
        • Comparison with OSCAR numbers and graph if possible.
        • Review of recommendations and responses.
        • Email copy to lead consultant.
      • Physician recommendations:
        • Deliver within three business days to Director of Nursing or designee.
        • Summary report of recommendations delivered to Director of Nursing, Administrator and any other person requesting within three business days.
        • Email copies to lead consultant. 
      • QA review of consultant activity:
        • Review of charts.
        • Review of recommendations.
        • Paper reviews from computer printout.
    • Maintain awareness of current laws, regulations, and guidelines for long term care facilities, both federal and state (where applicable and licensed).
    • Maintain awareness of Symbria RX Services policy and procedures.
    • Maintain awareness of developing trends and treatments of all aspects of long term care including, but not limited to; psychotropic drug reductions, psychotropic monitoring, appropriate drug use, and clinical monitoring.


    • Bachelor’s of Science degree in Pharmacy or Doctorate of Pharmacy (PharmD) from NABP accredited College of Pharmacy.
    • Valid state registered pharmacist license.
    • Possess knowledgeable understanding of current medication information, health care concepts and all regulations relative to the practices of institutional pharmacy.
    • General computer knowledge including Microsoft Office: Word, Excel and Outlook.
    • Exercise professional phone, verbal and written communication skills.
    • Ability to maintain organization, prioritize and multi-task. Exhibit initiative and reliability


    • Institutional pharmacy experience.
    • Previous experience utilizing RxPertise software
    • Basic knowledge of Docutrack and Framework LTC prescription processing.
