    HVA Senior Living Alliance, a company managed by Symbria, has an outstanding opportunity for a NEW Advanced Practice Registered Nurse (APRN) to help us launch a new business in northwest Pennsylvania. The primary purpose of the Advanced Practice Registered Nurse (APRN) is to provide advanced primary and/or specialty care and to serve patients of the group practice. The ideal candidate will be Board certified, experienced with Epic EHR System, and have APRN authorization to practice in Family Health or Gerontology by the Board of Nurse Examiners.

     

    This is an individual contributor role located in northwest Pennsylvania covering Meadville and Greenville geographic areas. The APRN will visit a variety of facilities and clients in the assigned area, and report to Symbria's Chief Medical Officer (CMO)

    For more than 20 years, HVA Senior Living Alliance has supported the program and service needs of senior living and post-acute consumers and organizations while helping thousands of older adults achieve and maintain their functional and wellness potential.

    Responsibilities

    • Provide primary health care to patients through direct care, consultation, collaboration, coordination and/or referral.
    • Perform physical exams and patient observations.
    • Create patient care plans and contribute to existing plans.
    • Order, administer and analyze diagnostic tests.
    • Monitor and operate medical equipment.
    • Diagnose health issues.
    • Detect changes in a patient’s health and change treatment plans as necessary.
    • Administer/Prescribe appropriate medicines and treatments.
    • Evaluate responses to treatments and medications.
    • Consult with healthcare professionals if/when needed.
    • Train patients and families on managing illnesses or injuries and disease prevention.
    • Keep and maintain appropriate records relating to all professional services including patient medical histories and symptoms.
    • Practice in accordance with the state’s laws, rules and regulations. Nurse Practitioner shall practice as a member of an interdisciplinary health care team for all patients.
    • Preparing and attending to, in connection with such services, all reports, claims, correspondence necessary or appropriate in the circumstances, all of which records, reports, claims and correspondence shall be the property of the Employer.
    • Performance of all things reasonably desirable and necessary to maintain and improve your professional skills.
    • Timely and accurate documentation of all patient visits and communication.
    • Timely and appropriate billing for services rendered.

    Qualifications

    • Master’s degree from an accredited nursing program.
    • Advanced Practice Registered Nurse License as per state requirement.
    • Board certified by either American Nurses Credentialing Center (ANCC) or American Association of Nurse Practitioners (AANP).
    • Prescriptive Authorization.
    • APRN authorization to practice in Family Health or Gerontology by the Board of Nurse Examiners.
    • Must be proficient with medical instruments and equipment.
    • Knowledge of computer-based data management programs and information systems, as well as electronic medical records.
    • Ability to communicate effectively, in verbal and written form, with retail and medical partners at various levels, patients, family members, physicians and representatives of the community.
    • Experience providing primary care as a family/gerontological nurse practitioner.
    • Sound understanding of all federal and state regulations including HIPAA and OSHA.
    • Knowledge of disease prevention.

    Experience preferred

    • Experience with Epic EHR system.
